I, the undersigned, hereby acknowledge that I am voluntarily participating in the Florida Youth At Risk Talent Show, hosted in partnership with Walmart. I understand that participation in this event involves inherent risks, including but not limited to physical injury, emotional distress, or property damage.
By signing this waiver, I agree to the following:
1. **Assumption of Risk:** I acknowledge that my participation in the talent show is voluntary and I assume all risks associated with my performance, including any risks that may arise from my actions or the actions of others.
2. **Release of Liability:** I hereby release, waive, and discharge Florida Youth At Risk, Walmart, their organizers, volunteers, sponsors, and any affiliated parties from any and all liability for any injury, loss, or damage that may occur as a result of my participation in the talent show.
3. **Indemnification:** I agree to indemnify and hold harmless Florida Youth At Risk, Walmart, and their affiliates from any claims or damages arising out of my participation in the event.
4. **Media Release:** I grant permission for Florida Youth At Risk and Walmart to use photographs and videos of my participation in this event for promotional purposes.
